Source: NTSB Chairman Mark V. Rosenker, on Tuesday's release of the Probable Cause report on the October 2004 'joyride' crash of a Pinnacle Airlines CRJ200. The board determined a laundry list of causes led to the accident, which claimed the lives of the two pilots onboard.
