NTSB Final Report: Bearhawk Patrol

Several Witnesses Reported Seeing The Pilot Out Of The Airplane At This Time

Analysis: Before the flight, the pilot told a friend that the airplane had a rigging issue that seemed to cause the airplane to “kick” laterally during turns. His friend performed a 10-minute flight and reported that he noticed the odd yawing moment while performing turns. The pilot responded that he would address the issue at a later time. After fueling the airplane, the pilot and pilotrated passenger fastened their seatbelts and departed the airport. Flight track information indicated that, several minutes after departure, the airplane leveled off. Soon thereafter, witnesses reported that the airplane “rolled” then “bucked” as its nose “dipped down initially,” then pitched up “quickly.”

“We would like to thank the many volunteers that help throughout the year to pull off the event, as well as the several reviewers, judges, and SURVICE staff that provide teams feedback on their deliverables and the opportunity to fly their aircraft competitively at the culminating event. Overall, the 4th Annual VFS DBVF competition was a monumental success, with teams working together to help each other overcome setbacks and push their aircraft to the limits. VFS looks forward to the 5th Annual DBVF competition, and for new university teams and supporters from around the world to join in on the fun.” Source: Dr. Jason K. Cornelius of NASA Ames Research Center, who volunteers as the VFS DBVF Program Director, commenting as the Vertical Flight Society Selected the Winners of this year's Design-Build-Vertical Flight Student Competition, sending the team from Texas A&M home with the gold.

NTSB Prelim: Piper Aircraft Inc PA-44-180

Controller Asked The Instructor If She Wanted To Declare An Emergency, And She Replied “Yes”

On March 30, 2024, about 1320 eastern daylight time, a Piper PA-44-180, N595ND, sustained substantial damage when it was involved in an accident at the Treasure Coast International Airport (FPR), Fort Pierce, Florida. The flight instructor was fatally injured, and the private pilot was seriously injured. The airplane was operated as a Title 14 Code of Federal  Regulations Part 91 instructional flight. The private pilot was training for his multi-engine rating at the time of the accident. He and the flight instructor departed FPR about 1140 and climbed to 5,000 ft mean sea level (msl), where they practiced single-engine emergency procedures. These procedures included shutting down and feathering the right engine. They returned to the airport and performed a simulated single-engine instrument “low” approach to runway 10R. To simulate the engine failure, thrust on the right engine was reduced to idle.
